I’ve been working on implementing two applications using MS List forms. Technically, the functionality is impressive, but I’ve encountered an issue that’s been frustrating in real-life use. Here's the problem:
I created a form and added its link to a SharePoint site. While it should be accessible to everyone in the organization, some users are randomly encountering an “You need permission to access this site” error when trying to open the form. Oddly, for some users, the error appears initially, but when they try again, the form opens without any issues.
This inconsistent behaviour has become a roadblock, and we’ve had to resort to using Microsoft Forms as a workaround. When I searched online, I found multiple forums discussing similar issues, like this one:

It’s clear that this isn’t an isolated incident, but I haven’t been able to pinpoint the root cause or find a definitive fix.
Has anyone else experienced this issue? Do you know what might be causing it and how to resolve it?

that sounds like a sharing permissions issue. SharePoint can be a bit of a minefield when it comes to sharing things with others. If your SharePoint site is made up of numerous things then you need to give access to ALL of those elements to the various people who need it. Sounds obvious or it might appear you have done this already, but it's likely you've given access to some bits and not all.

Yes, I have. I had been waiting for the attachments feature to finally roll out and updated my ticketing process to link to the form. I've had 3 or 4 issues with people requesting access to the site because they were denied permission. One user tried twice, both times she could submit, but told her there was an error so she submitted again. I think it was the attachment that threw the error as that was not included with the info. Not sure with the other two, they could submit at all and were prompted to request permission. Others have submitted without issue.
It's critical to get this fixed as I'm trying to steer people away from email. Super frustrating that it can't just work as expected. Tired of being a beta tester for Microsoft.
I face a similar issue too with one form embedded in a Sharepoint Page.
As a workaround I included a call to action button in top of the form with link to the form :)
